Who needs to submit proof of immunization?
All students born on or after January 1, 1957, need to provide proof of immunization or immunity to Measles, Mumps, and Rubella (MMR) and sign the Meningitis Response form found in the Health Information Form. Any student born before January 1, 1957, is required to submit proof of age and sign the Meningitis Response form. The meningitis vaccine is not mandatory, however, you are required to complete the form.

Students who are under 18 must have a parent/guardian answer/sign the meningitis Response on the Health Form.
It is important to know that while rare, Meningitis is a serious illness and college students are at an increased risk.

Where do I obtain my immunization records?
Records may be obtained from your high school health office, your local Public Health Department (if immunizations were received there), a previously attended college institution, military records, or from your own healthcare provider. A copy of your official immunization record may be submitted. If you are unable to obtain immunization records, please contact the Health and Wellness Center at 315-792-5452 to discuss options available to you.
Where do I submit my records?
Your records may be submitted to either the Utica Health and Wellness Center, located in the Alumni College Center, ACC 104, or the Rome Campus Student Services Center in the Plumley Complex 130. Records may also be faxed to 315-731-5854. You can also email a scanned signed copy to hcenter@mvcc.edu
Exemptions
MVCC recognizes medical and religious exemptions as defined by New York State Public Health Law 2165.
However, in the event of an outbreak of measles, mumps, or rubella, those students with medical and religious exemptions will be excluded from campus and classes to protect them from exposure for as long as they could be at risk.
Medical Exemptions
If a licensed physician, physician assistant, or nurse practitioner, or licensed midwife caring for a pregnant student certifies in writing that the student has a health condition which is a valid contraindication to receiving a specific vaccine, then a permanent or temporary (for resolvable conditions such as pregnancy) exemption may be granted. This statement must be on the practitioners letterhead, and must specify those immunizations which may be detrimental and the length of time they may be detrimental. Provisions need to be made to review records of temporarily exempted persons periodically to see if contraindications still exist. Religious Exemptions
A student may be exempt from vaccination if, in the opinion of the institution, that student or student's parent(s) or guardian of those less than 18 years old holds genuine and sincere religious beliefs which are contrary to the practice of immunization. The student requesting exemption may or may not be a member of an established religious organization. Requests for exemptions must be written and signed by the student if 18 years of age or older, or parent(s), or guardian if under the age of 18. The institution may require supporting documents. MVCC does not challenge a person's personal religious convictions. New York State does not recognize philosophical views as religious exemption; therefore, MVCC does not as well.
Internet Exemptions
MVCC reserves the right to make exemptions to on-line students who are taking six (6) or more credit hours and who are not required to be on campus in any group setting for reasons of testing, final exams, projects, etc.
